Acacia Hospice located in Burlington Township, NJ is currently hiring a Licensed Clinical Social Worker to work FULL TIME!

What will I do?

As a Social Worker you will provide psychosocial and emotional support to terminally ill patients and their families/caregivers. Responsible for developing, implementing and coordinating all social work interventions in the patient's plan of care. Oversees the management of volunteers with the assistance of the volunteer coordinator to whom the volunteers report to directly.

Our Social Worker is responsible for the following:

  • Assures that social service/counseling activities are integrated and coordinated within hospice.
  • Delivers specific, measurable, attainable plans and objectives, which enhance social service delivery.
  • Ability to perform a psychosocial assessment and reassessment on all terminally ill patients, including pediatric, adolescent, geriatric and patients in the general population and develop interventions in the plan of care.
  • Answers questions and offers emotional support to patient and family
  • Demonstrates knowledge of clinical social work.
  • Treats patients and their families with dignity and respect. Identifies and addresses psychosocial needs of patients and families.
  • Interacts professionally with patient/family and involves same in the formation of the plan of care.
  • Communicates appropriately and clearly to physicians, staff, Director of Clinical Services and team.
  • Coordinates social services to assure patient's needs are met and hospice policy is followed.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Manages and operates equipment safely and correctly.
  • Demonstrates the ability to be flexible, organized and functions under stressful situations.
  • Maintains a good working relationship both within the hospice and with others.
  • Consults with other disciplines as appropriate to collaborate in patient care and performance improvement activities.
  • Documentation meets professional standards and is timely.
  • Collaborates with the chaplain of Acacia Hospice and assists with the coordination of bereavement counseling and support programs.

  • What do I need for this role?
  • Masters Degree in Social Work from a graduate school of social work accredited by the Council on Social Work Education.
  • At least one year of postmaster's social work experience in a health care setting.
  • Current and valid Clinical Social Work license in the state of New Jersey (LSW / LCSW)
  • Hospice experience preferred
  • Current and valid Drivers License and Insurance in the state of New Jersey
  • Personal vehicle
