Masonic Village at Burlington located in Burlington Township, NJ is currently hiring a Full Time Social Worker! 

What will I do?

  • Provide supportive services to the residents in the Masonic Village that maximizes the resident's quality of life.  The employee is also an advocate, educator, counselor, mediator, assessor and discharge planner.  The employee ensures that residents' rights are protected in all situations.
  • Ensures that the community is meeting the social/emotional needs of the residents through participation in MDS evaluations, care planning, and the completion of other Social Services forms of documentation. This information will be included in the plan of care for each individual resident and will be periodically re-evaluated for current accuracy reflective of the resident with approaches to be followed to meet those needs identified.
  • Act as an advocate for resident rights by: orienting residents and staff about respecting the rights of residents, education on what constitutes abuse, the reporting and prevention of elder abuse.
  • Responsible for discharge planning arrangements and communication to and between various departments, residents and families in coordinating level of care changes and overseeing the processes that takes place during these transitions.
  • Discuss and educate residents and family/responsible party about Advance Directives, DNR, POLST, POA and other means for control over care. Assist residents with the execution and review of these forms.
  • Completes assessments on time and with accuracy on admission and during the residents stay.  Confers with the RNACs for accurate completion according to state and federal guidelines.
  • Communicates with team members regularly regarding problems, goals, plans, etc.  Holds team/ family conferences for all assigned residents. Problem solves resident issues.  Ensures timely follow up with team members, families, and residents to assure solutions were implemented and effective
  • Makes appropriate referrals.  Communicates to team, physician, and residents regarding changes in patient condition as appropriate
  • Responsible for federal, state, and local regulatory compliance.  Completes social services assessments and care planning in accordance with regulations. Completes timely and objective documentation on each resident according to state and federal regulations.
  • Provides social services consultation services, including but not limited to, counseling of new residents and family members in adjustment to the community; functional limitations and losses; crisis intervention; behavior; interpersonal conflicts.
  • Keeps informed of community services available to meet the social needs of all residents. Notifies appropriate department or outside resources of residents requiring assistance.
  • Acts as a liaison to families and advocate on behalf of residents. Maintains communication with resident's families to provide support as needed, monitors visiting patterns and addresses any concrete needs.
  • Provides social work supportive services, including but not limited to, understanding and exercising their rights; ensuring basic needs are met; educating residents and their families about the Medicaid process and assisting them in applying as needed; providing support and education groups; attending and participating in Resident Council meetings.

What do I need for this role?

  • Bachelor's Degree or Master's Degree in Social Work from an accredited college or university.
  • Certified or Licensed in Social Work by New Jersey Board of Social Work Examiners.
  • 1-year experience in a CCRC, Long-Term Care, Assisted Living/Comprehensive Personal Care Home or any combination of the above preferred.
  • At least 1-year experience in interviewing and counseling the elderly.
  • Good written, oral, and organizational skills.
  • Basic computer skills.
